Case Manager

Job description
We are seeking a computer savvy Case Manager to join our team at who will provide recovery-oriented rehabilitation services in group or individual settings. The successful candidate will be accountable for the full patient care cycle. Case manager duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the client’s health and human services needed. A certified case manager is a plus.


Responsibilities:

  • Create ISP’s, Discharge Plans, Annual Assessment for all new residents
  • Facilitate daily groups
  • Schedule and participate in monthly ART meetings with clinical and outpatient teams
  • Complete monthly CCR to identify resident care need and to justify stay
  • Implement and review treatment plans as needed
  • Collaborate with additional treatment providers and placement agencies
  • Ensure all documentation is completed in a timely manner
  • Coordinate and accompany residents to provider appointments

 

  • Skills / Requirements:
  • A High School Diploma/GED and three years of relevant experience in a behavioral health setting; OR
  • An associate degree and two years of relevant experience in a behavioral health setting; OR
  • A bachelor’s degree in the behavioral health field; OR
  • A non-behavioral health field bachelor’s degree and one year of relevant experience in a behavioral health setting
  • Must be at least 21 years of age.
  • Background check. Minimum typing speed of 25 wpm.
  • Some assignments require an Arizona Fingerprint Clearance Card
  • Current Arizona Driver License and a current satisfactory 39-month Motor Vehicle Record (MVR
